What do I need to bring to my appointment?

It is helpful to provide any previous scans, medical letters or reports to your appointment.

For all appointments, please bring any scans or reports from previous consultations with you. If you haven’t already provided us with this information we also require your GP’s details (name and address) and any other medical practitioner/​referrer who needs to be kept up to date with your progress.
